*Required for the consideration of any use of the facilities outside of worship services and regularly scheduled church-wide activities.

Once submitted your request will be reviewed by the ministerial staff and a member of the church staff will follow up.

*Must be age 21–75 to drive the church van. Ages 76+ are required by our insurance company to provide a doctor's release statement.
*Please Note: A driver's license is required for Background Check to drive, teach, OR chaperone any activity or event with minors.
All background checks must be renewed every 3 years.
Please bring:
- your government issued ID or driver's license OR provide a color copy of BOTH sides of your ID.
If submitting a Driver Background Check:
-In addition to the Driver Background Check Form, you must have passed a Calvary Background Check within the last 3 years.

*Calvary Med Forms are required for anyone who participates in off-site trips.
